Are you giving him whole milk? When my kids were about that age and totally addicted, we cut them down to two percent and, as soon as they stopped getting the bulk of their calories from it, the addiction was broken. (This, of course, leads to a story of how my oldest was actually allergic to dairy and it took us until he was four and had lost some hearing to figure it out... but that's another story...)
It's not the milk he's addicted to...it's the bottle!!
